LEPTOSPIROSIS

Penyakit Kencing Tikus atau Leptospirosis adalah penyakit yang bersifat zoonosis atau penyakit infeksius yang dapat ditularkan dari hewan ke manusia. Leptospirosis lebih umum terjadi di daerah tropis, tetapi juga ditemukan di daerah beriklim sedang seperti Eropa. 

Leptospirosis disebabkan oleh bakteri berbentuk spiral dari genus Leptospira, yang menginfeksi berbagai hewan liar dan hewan piaraan (Misalnya anjing dll). Leptospira bisa bersifat “patogen”, yang menyebabkan penyakit pada hewan atau manusia, atau bisa juga bersifat “saprophytic”, yang hidup bebas di permukaan perairan dan tidak (belum diketahui) menyebabkan penyakit.

Menurut WHO (2012), Leptospirosis adalah penyakit menular yang disebabkan oleh bakteri genus Leptospira. Leptospirosis terjadi di seluruh dunia, tetapi yang paling umum di daerah tropis dan subtropis. Wabah bisa terjadi setelah curah hujan yang berlebihan atau banjir.

Leptospirosis menginfeksi rodensia (hewan pengerat), hewan liar lainnya dan juga pada hewan peliharaan. Tikus (Rodensia) adalah hewan yang terlibat paling sering pada kasus manusia. Infeksi pada manusia masuk melalui lecet kulit, mukosa hidung, mulut dan mata. Terkena penyakit melalui air yang terkontaminasi oleh urin (air kencing) dari hewan yang terinfeksi adalah rute infeksi yang paling umum. Penularan dari manusia ke manusia jarang terjadi.

Pekerja di lapangan dan pertanian (Seperti pekerja sawah dan kebun tebu) sangat berisiko, tetapi juga ada sebuah recreational hazard (bahaya ikutan karena rekreasi) bagi mereka yang berenang atau yang mengarungi perairan yang terkontaminasi. Di daerah endemik jumlah kasus leptospirosis dapat mencapai puncaknya selama musim hujan dan bahkan dapat mencapai proporsi epidemi dalam kasus banjir karena banjir menyebabkan tikus pindah ke kota.

Pada manusia, gejala tahap awal penyakit ini termasuk di antaranya demam tinggi, sakit kepala parah, nyeri otot, menggigil, kemerahan pada mata, sakit perut, sakit kuning, pendarahan (hemoragi) di kulit dan selaput lendir, muntah, diare, dan ruam. (Sumber WHO).
Leptospirosis memiliki banyak nama yang berbeda, termasuk diantaranya: Weil's disease /Weil Syndrome (Penyakit Weil), 7-day fever (Demam 7 hari), harvest fever, Ricefield fever (Demam sawah), Canefield fever (Demam ladang tebu), Rat catcher's yellows, Fort Bragg fever, Pretibial fever, Black jaundice dan di Jepang disebut "nanukayami fever".

EPIDEMIOLOGI
Kejadian Penyakit
Kejadian penyakit Leptospirosis tidak pernah ada di Mesir, Greenland, Irak, dan Maladewa. Sedangkan kejadian terakhir penyakit Leptospirosis (definitif) di dunia adalah di negara: Azerbaijan November 2000, Belarus 2004, Belize November 2003, Bosnia dan Herzegovina Februari 2010, Bulgaria 2009, Cina Taipei 1991, Siprus 1983, Republik Ceko tahun 2005, Georgia tahun 1995, Yunani 2009, Grenada 2003, Islandia tahun 1995, Indonesia Maret 2011, Kuwait 1995, Kyrgyzstan Oktober 2010, Latvia pada Desember 2010, Malta tahun 2004, Moldova Desember 2004, Namibia 2000., Togo Juni 2010.

Hospes /Inang
Pada dasarnya semua mamalia rentan terhadap infeksi penyakit Leptospira yang patogenik, meskipun beberapa spesies tahan terhadap penyakit ini. Diantara umumnya hewan dan ternak, leptospirosis paling sering menginfeksi sapi, babi, anjing, dan kuda. Kucing tampaknya relatif tahan terhadap penyakit ini. Leptospirosis di satwa liar adalah hal yang biasa, namun penyakit ini paling sering diketahui hanya ketika satwa liar berfungsi sebagai sumber infeksi bagi hewan peliharaan atau manusia.

Penularan
Infeksi dapat diperoleh melalui kontak langsung atau tidak langsung dengan urine (air kencing) hewan yang terinfeksi, jaringan atau sekresi, atau air yang terkontaminasi dengan urine hewan yang terinfeksi. Leptospira masuk ke dalam tubuh melalui luka atau kulit yang rusak, tetapi juga dapat melintasi membran mukosa yang rusak ataupun yang utuh, dan mata. Penyebaran Orang ke orang sangat langka, atau tidak sama sekali. Kelompok berisiko utama untuk leptospirosis adalah buruh tani dan orang-orang yang memiliki kontak dengan yang berhubungan dengan rekreasi air misalnya pengayuh kano, penyelam, nelayan, peselancar dan mereka yang berenang di danau atau sungai. Sekitar setengah dari infeksi biasanya diperoleh melalui kegiatan rekreasi. Leptospirosis dapat diperoleh melalui kontak dengan tikus (peliharaan).

ETIOLOGI
Leptospira adalah bakteri yang bersifat aerobik, gram negatif spirochetes, lambat tumbuh, dan memiliki karakteristik motilitas seperti pembuka botol (corkscrew-like motility). Taksonomi dari Leptospira adalah komplek dan bisa membingungkan. Secara tradisional, Leptospira dibagi menjadi 2 kelompok; 1. Leptospira Patogenik yang diklasifikasikan sebagai anggota L interrogans dan 2. Leptospira Saprophytik diklasifikasikan sebagai L biflexa. Dalam masing-masing spesies ini, serovar-serovar leptospiral diakui, dengan ~ 220 serovar berbeda dari Leptospira Patogenik diidentifikasi (berdasarkan antigen permukaan) di seluruh dunia. Dengan meningkatnya penggunaan informasi genom untuk klasifikasi bakteri, genus Leptospira direorganisasi dengan leptospira patogen sekarang diidentifikasi dalam 7 spesies Leptospira. Beberapa leptospiral patogen umum pada hewan peliharaan sekarang memiliki nama spesies yang berbeda. Sebagai contoh, L interrogans serovar Grippotyphosa sekarang L kirschneri serovar Grippotyphosa. Dua (2) jenis serovar hardjo telah resmi dibagi menjadi 2 jenis: jenis serovar hardjo hardjo-bovis (ditemukan di Amerika Serikat dan sebagian besar dunia) sekarang L borgpetersenii serovar hardjo dan kurang umum jenis serovar hardjo hardjo-Prajitno (ditemukan terutama di Inggris) kini L interrogans serovar hardjo. Nomenklatur yang direvisi sekarang tercermin dalam literatur ilmiah tetapi tidak pada label untuk vaksin dan produk farmasi. Nama-nama serovar tetap dan berguna ketika membahas epidemiologi, gambaran klinis, pengobatan, dan pencegahan leptospirosis.

Menurut Ron Hines 2014, terdapat sangat banyak leptospira. Saat ini, sekitar 230 di antaranya telah diidentifikasi. Mereka dibagi menjadi strain (atau serovar), berdasarkan karakteristik protein permukaan mereka. Delapan di antaranya diketahui menyebabkan penyakit pada anjing, kucing dan hewan lainnya. Mereka adalah: Leptospira icterohaemorrhagiae, L. canicola, L. Grippotyphosa, L. pomona, L. bratislava, L. automnalis, L. bataviae, L. hardjo, dan L. Grippotyphosa. Empat leptospira yang pertama adalah yang paling umum menginfeksi anjing.

Klasifikasi Agen Penyebab Penyakit
Kingdom: Bakteri, Phylum: Spirochaetes, Class: Spirochaeates, Ordo: Spirochaetales, Familia: Leptospiraceae, Genus: Leptospira.

Ketahanan Terhadap Tantangan Fisik Dan Kimia
1. Temperatur: Suhu pada atau di atas 131 °F (42 °C) membunuh leptospira.
2. pH: Bisa bertahan hidup pada pH di bawah 7,0 berkisar antara 10 - 117 hari, dan pada pH lebih dari 7,0 berkisar antara 21 - 152 hari.
3. Desinfektan: Semua desinfektan rumah tangga biasa (pemutih, produk berbasis alkohol) membunuh leptospira secara cepat; seperti halnya aplikasi deterjen atau merebus selama 5 menit. Genangan air dapat didesinfeksi menggunakan tablet klorin untuk kolam renang (tapi harus sadar bahwa produk tersebut adalah racun bagi kehidupan air).
4. Ketahanan hidup: Leptospira sangat tergantung pada air, lumpur atau tanah liat basah untuk bertahan hidup. Itu karena mereka tidak memiliki membran tahan air untuk melindungi mereka dari keadaan kering. Leptospira segera mati pada permukaan kering.

DIAGNOSA
Penggunaan, interpretasi, dan nilai dari prosedur diagnostik laboratorium untuk leptospirosis bervariasi dengan sejarah klinis dari hewan atau ternak, lama infeksi, dan serovar yang menginfeksi. Leptospirosis akut harus dicurigai pada kasus berikut: Serangan mendadak agalactia (pada sapi dan domba menyusui); ikterus dan hemoglobinuria, terutama pada hewan muda; meningitis; dan gagal ginjal akut atau penyakit kuning pada anjing. Leptospirosis kronis harus dipertimbangkan dalam kasus berikut: abortus, lahir mati, lahir lemah (mungkin terlalu dini); infertilitas; gagal ginjal kronis atau hepatitis aktif kronis pada anjing; dan kasus-kasus ophthalmia periodik pada kuda. Dua hal utama kosekuensi mikrobiologi dari infeksi Leptospira menunjukan masalah diagnostik tertentu: lokalisasi dan persistensi dari leptospira pada ginjal dan dalam saluran kelamin jantan betina. Hewan yang terinfeksi kronis mungkin menjadi hewan carier selama bertahun-tahun untuk hidup dan berfungsi sebagai reservoir infeksi untuk hewan lainnya dan manusia.

Gejala
Tanda-tanda hewan teserang atau gejala klinis, bervariasi tergantung pada spesies hewan yang terkena, dan jenis organisme leptospiranya.

Gejala umum penyakit ini antara lain: Demam, anorexia (tidak ada nafsu makan), depresi, anemia, dispnea (susah bernafas /sesak nafas).

Gejala khusus adalah: Abortus, lahir mati dan bayi lahir lemah, penurunan produksi susu dan mastitis, ikterus (= jaundice), hemoglobinuria, konjungtivitis, pada kuda ini adalah gejala utama dan dapat berkembang untuk keratitis, hypopyon dan diikuti kebutaan. Kematian pada hewan piaraan biasanya tidak melebihi 5%, tetapi dapat mencapai 20% pada ruminansia kecil dan bahkan lebih tinggi terjadi pada anjing. Angka kematian pada hewan muda lebih tinggi.

Gejala klinis Leptospirosis pada Anjing adalah: Demam, nyeri sendi atau otot, ini dapat termanifestasi sebagai keengganan untuk bergerak, nafsu makan menurun, lemah, muntah dan diare, adanya cairan yang keluar dari hidung dan mata, sering buang air kecil, bisa diikuti oleh kurangnya buang air kecil, menguningnya warna  gusi, membran di sekitar mata, dan kulit (jaundice).

Leptosira hardjo pada sapi dapat menyebabkan produksi susu turun mendadak, aborsi, kelahiran anak sapi lemah atau infertilitas. Babi dengan Leptospira serovar bratislava, menyebabkan penyakit reproduksi, namun pentingnya infeksi ini pada babi masih belum jelas.
Beberapa hewan mungkin tetap pembawa (carier) organisme ini tanpa menunjukkan tanda-tanda klinis dan tetap (selalu) menjadi sumber infeksi bagi yang lain.

Menurut F Alexander (2014) dari FAO, Gejala gejala klinis Leptospirosis pada manusia biasanya non spesifik (gejala flu) dan penyakit ini mungkin tidak terdeteksi. Gejala pertama meliputi: Demam tinggi, sakit kepala, menggigil, nyeri otot, Mual, muntah dan diare. Jika pasien tidak sembuh pada tahap ini, kondisi dapat memburuk dan gejala berikut dapat terlihat: Sakit kepala, nyeri otot, perembesan konjungtiva, Limfadenopati, ruam, meningitis, gagal ginjal, gagal hati, hemoragi (perdarahan), miokarditis, ikterus (penyakit Weil). Tingkat kematian dapat berkisar 5 sampai 40% sesuai dengan serovar yang menginfeksi dan status kekebalan pasien.

Lesi
Temuan lesi post-mortem (setelah kematian) pada semua spesies mungkin muncul sebagai berikut: ikterus, Hemoglobinuria, hemoragi (perdarahan) submukosa, ginjal bengkak dengan petekie multifokal dan hemoragi ekimotiK, kadang membentuk bintik-bintik putih, limphonodus (kelenjar getah bening) yang bengkak dan hemoragi, mumifikasi janin.

Diagnosa Banding
Pada sapi diagnosa /diagnosis banding Leptospirosis meliputi: Babesiosis; anaplasmosis; Hemoglobinuria Postparturient; anemia hemolitik akut anak sapi; penyebab lain dari aborsi dan agalactiae (brucellosis, demam Q, IBR, BVD, dst).

Pada babi diagnosa banding meliputi: Brucellosis; Erysipelas; Infeksi Parvovirus; Porcine Reproductive and Respiratory Syndrome; Aujeszky's disease; Hog cholera.
Pada ruminansia kecil diagnosa banding meliputi: Keracunan tembaga (copper) kronis; Anaplasmosis.

Pada kuda diagnosa banding meliputi: Infectious Equine Anemia; Equine Viral Rhinopneumonitis; Equine Viral Artheritis; Streptococcus zooepidemicus; Salmonella abortus equi.

Pada anjing diagnosa banding meliputi: Ehrlichiosis; Autoimmune hemolytic anemia; Infectious canine hepatitis virus; Canine brucellosis; Canine herpesvirus infection; Canine distemper; Hepatitis; Piroplasmosis.

Pada manusia diagnosa banding meliputi: Dengue fever or other hemorrhagic fever; Viral hepatitis; Hantivirus disease; Viral meningitis; "Viral syndrome".

Diagnosa Laboratorium
Diagnosa /diagnosis laboratorium terhadap leptospirosis bisa komplek, dan meliputi uji yang terbagi dalam dua kelompok. Satu kelompok uji dirancang untuk mendeteksi antibodi anti leptospira dan kelompok lain dari uji dirancang untuk mendeteksi leptospira, antigen leptospira, atau asam nukleat leptospira pada jaringan atau cairan tubuh hewan. Cara pengujian khusus yang dipilih tergantung pada tujuan pengujian (Misalnya survey kawanan atau pengujian hewan individual) dan pada uji atau kemampuan uji yang ada di daerah.

Sampel:
Sampel yang digunakan diantaranya: Organ internal (dalam), urine (air kencing), cairan tubuh.
Identifikasi Agen Penyakit:

Isolasi atau penampilan dari leptospira di: 1. organ internal (seperti hati, paru-paru, otak, dan ginjal) dan cairan tubuh (darah, susu, serebrospinal, dada dan cairan peritoneal) hewan klinis terinfeksi memberikan diagnosis definitif dari penyakit klinis yang bersifat akut atau, dalam kasus pada fetus (janin), infeksi kronis dari induknya; 2. ginjal, urin, atau saluran genital hewan tanpa tanda-tanda diagnosis klinis hanya karier.

Isolasi leptospira dari material klinis dan identifikasi isolat memakan waktu dan tugas bagi laboratorium rujukan khusus. Isolasi diikuti oleh penipean dari ginjal karier adalah penting dan sangat berguna dalam studi epidemiologi untuk menentukan keberadaan serovar dalam kelompok tertentu hewan, spesies hewan, atau wilayah geografis.

Keberadaan leptospira dengan uji immunokimia (immunofluorescence dan immunohistokimia) lebih sesuai untuk kebanyakan laboratorium. Namun, efektivitas dari tes ini tergantung pada jumlah organisme yang ada di dalam jaringan, dan tes ini tidak memiliki sensitivitas kultur. Kecuali digunakan reagen khusus, uji immunokimia tidak mengidentifikasi serovar infektor dan hasilnya harus diinterpretasikan dalam hubungannya dengan hasil uji serologi.

Reagen untuk imunofluoresensi terbaik disiapkan dengan sera anti-leptospira IgG titer tinggi, yang tidak tersedia secara komersial. Serum tipe-leptospira kelinci atau antibodi monoklonal dapat digunakan untuk imunohistokimia dan ada di laboratorium rujukan leptospira.

Materi genetik dari leptospira dapat ditunjukkan dalam jaringan atau cairan tubuh dengan menggunakan berbagai uji berdasar pada polymerase chain reaction (PCR), baik secara real-time atau secara tradisional. Uji PCR sensitif, tetapi prosedur pengendalian mutu dan pengolahan sampel untuk PCR sangat penting dan harus disesuaikan dengan jaringan, cairan dan spesies yang diuji. Seperti uji immunokimia, uji PCR tidak mengidentifikasi serovar infektor.

Tes /Uji Serologi:
Pengujian serologis adalah cara yang paling banyak digunakan untuk mendiagnosis leptospirosis, dan Microscopic Agglutination Test (MAT) /Uji aglutinasi mikroskopis adalah Uji serologi standar. Antigen yang dipilih untuk digunakan dalam MAT harus mencakup strain yang merepresentasikan dari serogrup yang diketahui ada di wilayah tertentu ditambah yang diketahui sebagai serogrup yang dipertahankan di tempat lain oleh spesies inang yang diuji.

MAT ini digunakan untuk menguji hewan individual dan hewan kawanan. Sebagai hewan uji individu, MAT ini sangat berguna untuk mendiagnosis infeksi akut: kenaikan empat kali lipat titer antibodi dalam sampel serum akut berpasangan dan sampel serum konvalesen adalah diagnostikanya. Untuk menghasilkan informasi yang berguna dari kawanan hewan, setidaknya sepuluh hewan, atau 10% dari kawanan, mana yang lebih besar yang seharusnya diuji dan sejarah vaksinasi hewan yang telah didokumentasikan.

MAT memiliki keterbatasan dalam diagnosis infeksi kronis pada hewan individu dan dalam diagnosis infeksi endemik pada hewan kawanan /ternak. Hewan yang terinfeksi barangkali gagal atau menjadi carier (pembawa) penyakit genital /ginjal dengan titer MAT di bawah titer minimum yang signifikan yang diterima secara luas yaitu 1/100 (pengenceran akhir).

Uji Enzyme-Linked Immunosorbent Assays (ELISAs) juga dapat digunakan untuk mendeteksi antibodi terhadap leptospira. Sejumlah tes telah dikembangkan dan terutama digunakan untuk mendeteksi infeksi baru dan uji skreening untuk hewan percobaan untuk dipakai dalam studi tantangan. Hewan yang telah divaksinasi terhadap serovar mungkin positif dalam beberapa uji ELISA, sehingga menyulitkan interpretasi dari hasil uji.

PENCEGAHAN DAN PENANGGULANGAN
Pencegahan Dengan Sanitasi
Langkah-langkah kontrol /pengendalian Rodensia (Tikus) mengurangi kemungkinan adanya infeksi, dan di daerah di mana Leptospirosis adalah umum (biasa terjadi), mencegah hewan (anjing dll) dari bermain /berenang di kolam dan air yang bergerak lambat juga dapat membantu. Jika hewan peliharaan Anda telah didiagnosis dengan Leptospirosis, langkah-langkah untuk mencegah infeksi (penularan) meliputi: Hindari kontak dengan urine jika mungkin, dan gunakan pakaian pelindung (sarung tangan, dll) jika Anda perlu untuk menangani urine; Praktek kebersihan yang baik termasuk mencuci tangan hati-hati; Disinfeksi permukaan tempat di mana hewan peliharaan yang terinfeksi telah kencing (disinfektan antibakteri atau larutan pemutih yang diencerkan); Ikuti saran dokter hewan untuk pengobatan dan pastikan semua obat yang diberikan sesuai petunjuk.

Pencegahan Dan Pengobatan Secara Medis
Pengobatan Leptospirosis:
Antibiotik biasa digunakan untuk membunuh bakteri Leptospira dan sering diberikan dalam dua tahap: satu jenis antibiotik diberikan untuk mengobati infeksi awal, kemudian diikuti dengan pemberian berbagai jenis antibiotik untuk memerangi shedding /penumpahan bakteri dalam urin. Treatmen (pengobatan) dimulai lebih awal semakin baik hasilnya.

Setelah gejala gagal ginjal dan /atau gagal hati terlihat, prognosa /prognosis untuk kemungkinan sembuh lebih buruk. Dalam kasus ini, pengobatan agresif sangat penting, termasuk pemberian infus /cairan intravena, obat-obatan untuk mengurangi muntah dan mengobati efek lain dari gagal ginjal dan gagal hati, dan dialisis. Namun demikian, tergantung pada tingkat keparahan penyakit, pengobatan tidak selalu berhasil ketika gejala kegagalan organ telah nampak.

Pencegahan Leptospirosis:
Vaksin untuk melawan leptospirosis tersedia dan direkomendasikan di daerah di mana penyakit leptospirosis biasa terjadi. Vaksin hanya diproduksi untuk varietas tertentu dari Leptospira, dan tidak menawarkan kekebalan yang tahan lama, sehingga perlu sering diulang.
Meskipun vaksin tidak 100 persen efektif dan tidak melindungi terhadap semua jenis Leptospira, vaksinasi masih tetap dianjurkan untuk membantu mencegah penyakit yang berpotensi serius yang dapat menular kepada manusia. Vaksin yang disarankan dan jadwal vaksinasi harus didiskusikan dengan dokter hewan bila akan diberikan berdasarkan faktor faktor risiko pada hewan (anjing) yang bersangkutan.

LEPTOSPIROSIS

Leptospirosis or Penyakit Kencing Tikus is a zoonosis - an infection that can be transmitted from animals to humans. It is more common in tropical areas of the world but is also found in temperate areas such as Europe, including the United Kingdom (UK). Leptospirosis is caused by spiral-shaped bacteria of the genus Leptospira (referred to as leptospires), which infect a variety of wild and domestic animals. Leptospires maybe pathogenic, which cause disease in animals or humans, or saprophytic, which are free-living in surface waters and are not known to cause disease.
According WHO (2012), Leptospirosis is an infectious disease caused by bacteria belonging to the genus Leptospira. Leptospirosis occurs worldwide, but is most prevalent in tropical and subtropical regions. Outbreaks can occur following excessive rainfall or flooding.
Leptospirosis is an infection in rodents and other wild and domesticated species. Rodents are implicated most often in human cases. The infection in man is contracted through skin abrasions and the mucosa of the nose, mouth and eyes. Exposure through water contaminated by urine from infected animals is the most common route of infection. Human-to-human transmission is rare.
Outdoor and agricultural workers (rice-paddy and sugarcane workers for example) are particularly at risk but it is also a recreational hazard to those who swim or wade in contaminated waters. In endemic areas the number of leptospirosis cases may peak during the rainy season and even may reach epidemic proportions in case of flooding because the floods cause rodents to move into the city.
In human the early stages of the disease, symptoms include high fever, severe headache, muscle pain, chills, redness of the eyes, abdominal pain, jaundice, haemorrhages in the skin and mucous membranes, vomiting, diarrhoea, and rash. (WHO).
Leptospirosis has many different names including: Weil's disease /Weil Syndrome (Penyakit Weil), 7-day fever (Demam 7 hari), harvest fever, Ricefield fever (Demam pesawah), Canefield fever (Demam ladang tebu), Rat catcher's yellows, Fort Bragg fever, Pretibial fever, Black jaundice and in Japan it is called "nanukayami fever".

EPIDEMIOLOGY
Occurence 
Leptospirosis disease never occurred in Egypt, Greenland, Iraq, and Maldives. Hosts 
Essentially all mammals are susceptible to infection with pathogenic Leptospira, although some species are more resistant to disease. Among common companion animals and livestock, leptospirosis is most frequently recognized in cattle, swine, dogs, and horses. Cats seem to be relatively resistant to disease. Leptospirosis in wildlife is common, although the disease is most often noticed only when the wildlife serve as a source of infection for domestic animals or humans.

Transmission 
Infection may be acquired by direct or indirect contact with infected animal urine, tissues or secretions, or water contaminated with infected animal urine. Leptospires enter the body through cut or damaged skin, but may also pass across damaged or intact mucous membranes, and the eyes. Person-to-person spread is extremely rare, if it occurs at all. The main risk groups for leptospirosis in the UK are farm workers and people who have recreational contact with water eg canoeists, divers, fishermen, windsurfers, potholers and those who swim in lakes or rivers. About half of indigenous infections are acquired through recreational activities. Leptospirosis can be acquired through contact with pet rats.

AETIOLOGY 
Leptospira are aerobic, gram-negative spirochetes that are fastidious, slow growing, and have characteristic corkscrew-like motility. The taxonomy of Leptospira is complex and can be confusing. Traditionally, Leptospira were divided into 2 groups; the pathogenic Leptospira were all classified as members of L interrogans and the saprophytic Leptospira were classified as L biflexa. Within each of these species, leptospiral serovars were recognized, with ~220 different serovars of pathogenic Leptospira identified (based on surface antigens) throughout the world. With the increased use of genomic information for the classification of bacteria, the genus Leptospira was reorganized with the pathogenic leptospires now identified in 7 species of Leptospira. Some of the common leptospiral pathogens of domestic animals now have different species names. For example, L interrogans serovar grippotyphosa is now L kirschneri serovar grippotyphosa. The 2 types of serovar hardjo have been formally split into 2 species: serovar hardjo type hardjo-bovis (found in the USA and much of the world) is now L borgpetersenii serovar hardjo and the less common serovar hardjo type hardjo-prajitno (found primarily in the UK) is now L interrogans serovar hardjo. The revised nomenclature is now reflected in the scientific literature but not on labels for vaccines and pharmaceutical products. The serovar names remain and are useful when discussing the epidemiology, clinical features, treatment, and prevention of leptospirosis.
According Ron Hines 2014, There are a very large number of leptospira. Currently, about 230 of them have been identified. They are divided into strains (or serovars), based on the characteristic of their surface proteins. Eight of these are known to cause disease in dogs, cats and ather animals. They are: Leptospira icterohaemorrhagiae, L. canicola, L. grippotyphosa, L. pomona, L. bratislava, L. automnalis, L. bataviae, L. hardjo, and L. grippotyphosa . The first four are the most common ones that infect dogs.

Classification Of The Causative Agent 
Kingdom: Bacteria, Phylum: Spirochaetes, Class: Spirochaeates, Ordo: Spirochaetales, Familia: Leptospiraceae, Genus: Leptospira.

Resistance To Physical And Chemical Action 
1. Temperature: Temperatures at or above 131F (42C) kill leptospira as well.
2. pH: Survival at pH under 7.0 ranged from 10 to 117 days and at pH over 7.0 from 21 to 152 days.
3. Disinfectants: All common household disinfectants (bleaches, alcohol based products, vinegar, lemon juice etc. Porous items need to be completely submerged in solutions) kill leptospira quickly ; as does a liberal application of detergent or boiling for 5 minutes. Standing water can be disinfected using swimming pool chlorine tablets (but realize that those products are toxic to aquatic life).
4. Survival: Leptospira are very dependent on water, mud or damp clay soils to survive. That is because they do not possess a waterproof membrane to protect them from drying. Leptospira die almost immediately on dry surfaces.

DIAGNOSIS 
The use, interpretation, and value of laboratory diagnostic procedures for leptospirosis vary with the clinical history of the animal or herd, the duration of infection, and the infecting serovar. Acute leptospirosis should be suspected in the following cases: sudden onset of agalactia (in adult milking cattle and sheep); icterus and haemoglobinuria, especially in young animals; meningitis; and acute renal failure or jaundice in dogs. Chronic leptospirosis should be considered in the following cases: abortion, stillbirth, birth of weak offspring (may be premature); infertility; chronic renal failure or chronic active hepatitis in dogs; and cases of periodic ophthalmia in horses. Two major chronic microbiological sequelae of leptospiral infection present particular diagnostic problems: the localisation and persistence of leptospires in the kidney and in the male and female genital tract. Chronically infected animals may remain carriers for years to life and serve as reservoirs of the infection for other animals and humans.

Signs 
Clinical signs vary depending on the species of animal affected, and the type of leptospiral organism involved.
General signs of the disease include: Fever, Anorexia, Depression, Anemia, Dyspnea.
More specific signs are: Abortions, stillbirths and weak newborns, Drop in milk yield and mastitis, Icterus (= jaundice), Hemoglobinuria, Conjunctivitis, in horses it is a major symptom and can evolve to keratitis, hypopyon and eventually blindness. Mortality in domestic animals does not usually exceed 5% but may reach 20% in small ruminants and even higher in dogs. Mortality is higher in young animals.
Clinical signs Leptospirosis in Dogs, Fever, joint or muscle pain - this may manifest as a reluctance to move, decreased appetite, weakness, vomiting and diarrhea, discharge from nose and eyes, frequent urination - may be followed by lack of urination, yellowing of the gums, membranes around the eyes, and skin (jaundice).
Leptosira hardjo in cattle may cause a sudden drop in milk production, abortions, birth of weak calves or infertility. Pigs with Leptospira serovar bratislava, which is reported to cause reproductive disease. However the significance of this infection in pigs remains uncertain.
Some animals may remain carriers of the organism without showing clinical signs and remain a source of infection for others.
In humans the clinical signs are usually non specific (flu-like symptoms) and the disease may remain undetected. The first symptoms include: High fever, Headaches, Chills, Muscle pains, Nausea, vomiting and diarrhoea.  If the patient does not recover at this stage, the condition can deteriorate and the following signs can be seen: Headaches, Muscle tenderness, Conjunctival suffusion, Lymphadenopathy, Rash, Meningitis, Kidney failure, Liver failure, Hemorrhage, Myocarditis, Icterus (Weil's disease). Mortality rate can range from 5 to 40% according to the serovars involved and the patient's immunological status.

Lesions 
Post-mortem findings. In all species the following lesions may appear: Icterus, Hemoglobinuria, Submucosal haemorrhages, Kidneys are swollen with multifocal petechial and ecchymotic hemorrhages, forming sometimes white spots, Lymph nodes are swollen and hemorrhagic, Mummified foetus.

Differential Diagnosis 
In cattle the differential diagnosis include: Babesiosis; Anaplasmosis; Post-parturient hemoglobinuria; acute hemolytic anemia of calves; other causes of abortion and agalactiae (brucellosis, Q fever, IBR, BVD, etc).
In pigs the differential diagnosis include: Brucellosis; Erysipelas; Parvovirus infection; Porcine Reproductive and Respiratory Syndrome
Aujeszky's disease; Hog cholera.
In small ruminants the differential diagnosis include: Chronic copper poisoning; Rape poisoning; Anaplasmosis.
In horses the differential diagnosis include: Infectious Equine Anemia; Equine Viral Rhinopneumonitis; Equine Viral Arteritis; Streptococcus zooepidemicus; Salmonella abortus equi.
In dogs the differential diagnosis include: Ehrlichiosis; Autoimmune hemolytic anemia; Infectious canine hepatitis virus; Canine brucellosis; Canine herpesvirus infection; Canine distemper; Hepatitis; Piroplasmosis.
In humans the differential diagnosis include :Dengue fever or other hemorrhagic fever; Viral hepatitis; Hantivirus disease; Viral meningitis; "Viral syndrome".

Laboratory Diagnosis 
Laboratory diagnosis of leptospirosis can be complex and involves tests which fall into two groups. One group of tests is designed to detect antileptospiral antibodies and the other group of tests is designed to detect leptospires, leptospiral antigens, or leptospiral nucleic acid in animal tissues or body fluids. The particular testing regimen selected depends on the purpose of testing (e.g. herd surveys or individual animal testing) and on the tests or expertise available in the area.
Samples:
Internal organs, urine, body fluids.
Identification Test Of The Agent:
The isolation or demonstration of leptospires in: 1. the internal organs (such as liver, lung, brain, and kidney) and body fluids (blood, milk, cerebrospinal, thoracic and peritoneal fluids) of clinically infected animals gives a definitive diagnosis of acute clinical disease or, in the case of a fetus, chronic infection of its mother; 2. the kidney, urine, or genital tract of animals without clinical signs is diagnostic only of a chronic carrier state.
Isolation of leptospires from clinical material and identification of isolates is time-consuming and is a task for specialised reference laboratories. Isolation followed by typing from renal carriers is important and very useful in epidemiological studies to determine which serovars are present within a particular group of animals, an animal species, or a geographical region.
The demonstration of leptospires by immunochemical tests (immunofluorescence and immunohistochemistry) is more suited to most laboratory situations. However, the efficacy of these tests is dependent on the number of organisms present within the tissue, and these tests lack the sensitivity of culture. Unless specially prepared reagents are used, immunochemical tests do not identify the infecting serovar and results must be interpreted in conjunction with serological results.
Reagents for immunofluorescence are best prepared with high IgG titre anti-leptospire sera, which are not available commercially. Rabbit leptospiral-typing serum or monoclonal antibodies can be used for immunohistochemistry and are available from leptospiral reference laboratories.
Genetic material of leptospires can be demonstrated in tissues or body fluids using a variety of assays based on the polymerase chain reaction (PCR), either in real-time or traditional formats. PCR assays are sensitive, but quality control procedures and sample processing for PCR are critical and must be adjusted to the tissue, fluid and species being tested. Like immunochemical tests, PCR assays do not identify the infecting serovar.
Serological Tests:
Serological testing is the most widely used means for diagnosing leptospirosis, and the microscopic agglutination test (MAT) is the standard serological test. Antigens selected for use in the MAT should include representative strains of the serogroups known to exist in the particular region plus those known to be maintained elsewhere by the host species under test.
The MAT is used to test individual animals and herds. As an individual animal test, the MAT is very useful for diagnosing acute infection: a four-fold rise in antibody titres in paired acute and convalescent serum samples is diagnostic. To obtain useful information from a herd of animals, at least ten animals, or 10% of the herd, whichever is greater, should be tested and the vaccination history of the animals documented.
The MAT has limitations in the diagnosis of chronic infection in individual animals and in the diagnosis of endemic infections in herds. Infected animals may abort or be renal/genital carriers with MAT titres below the widely accepted minimum significant titre of 1/100 (final dilution).
Enzyme-linked immunosorbent assays (ELISAs) can also be useful for detection of antibodies against leptospires. Numerous assays have been developed and are primarily used for the detection of recent infections and the screening of experimental animals for use in challenge studies. Animals that have been vaccinated against the serovar of interest may be positive in some ELISAs, thus complicating interpretation of the results.

PREVENTION AND CONTROL
Sanitary Prophylaxis 
Rodent control measures reduce the chances of infection, and in areas where Leptospirosis is common, preventing animals (dogs etc) from swimming in ponds and slow-moving water can also help. If your pet has been diagnosed with Leptospirosis, steps to prevent infections include the following: Avoid contact with urine if possible, and wear protective clothing (gloves, etc.) if you need to handle urine; Practice good hygiene including careful hand washing; Disinfect surfaces where infected pets have urinated (antibacterial disinfectant or diluted bleach solution); Follow your vet's advice for treatment and make sure all medications are given as directed.

Medical Prophylaxis 
Treating Leptospirosis:
Antibiotics are used to kill Leptospira bacteria and are often given in two stages: one type of antibiotic to treat the initial infection, followed up with a different kind of antibiotic to combat the shedding of bacteria in the urine. The earlier treatment is started, the better.
Once kidney and/or liver failure is present, the prognosis for recovery is worse. In these cases, aggressive treatment is vital, including intravenous fluids, medications to reduce vomiting and treat other effects of kidney and liver failure, and dialysis. However, depending on the severity of disease, treatment is not always successful when organ failure is present.
Preventing Leptospirosis:
Vaccines against leptospirosis are available and recommended in areas where leptospirosis is common. The vaccines are only produced for a few specific varieties of Leptospira, and don't offer long-lasting immunity, so need to be repeated often.
Although the vaccines are not 100 percent effective and do not protect against all types of Leptospira, vaccination is still recommended to help prevent a potentially serious disease that can be transmitted to people. Recommended vaccines and vaccination schedules should be discussed with your vet based on your dog's risk factors.
